Hobart Diabetes Health is a private practice and therefore charges a fee for all consultations. The fee must be paid on the day of your appointment.
You do not need a referral from your GP to book an appointment however, you may wish to ask your GP for a ‘Chronic Disease Management Plan’ (Item 721) or ‘Team Care Arrangement’ (Item 723) that provides a Medicare rebate of $60.30 for a maximum of 5 Allied Health Care visits per year.
If you have private health insurance, you may be able to claim a rebate from your insurer. Please enquire with your health fund directly for further information.
If you cannot make your appointment, please reschedule or cancel your appointment as soon as possible. If you fail to attend your appointment, reschedule or cancel within 24 hours of your booking, a 20% cancellation fee will apply.
