For Intake Enquiries:
1. Download a copy of our Referral Form from our website and email this to intake@abilitypsychologyservices.com (or alternatively, submit this through our website).
2. Anticipate a call from a private number and allow approx. 20 minutes to complete the Intake. If you miss our call, you can return contact on 1300 229 906 and select Option 2 from the menu to speak with our Intake Clinician, Tamara.
2. Come prepared with relevant information, including:
- Reason for referral (i.e., goal).
- Location of client and general availability.
- Relevant information about their individual circumstances that may inform referral.
- Relevant history, such as former psychological support/s accessed and outcome.
- Individual preferences, such as preferred appointment frequency (e.g., weekly / fortnightly), and preferred modality (e.g., face-to-face, telehealth (videoconference only), or both). This information can be very helpful when determining mutual availability should a referral progress to booking.
3. If there are any assessment needs (e.g., an NDIS review date within <6 months), please notify us at Intake so that we can schedule any necessary assessments and allow sufficient time to form any required reports to support review, within scope.
Information discussed is only ever broad, and is used to determine if our services may be suitable to meet the identified support needs. It also helps us to endeavour to match the individual with a suitably experienced Clinician in their local area.
